Millennium Team Benefits Summary
HEALTH CARE
Each Team member offers a variety of HMO, PPO, and POS health care options. The level of benefit provided will vary depending on the choice of provider. Employees are requested to contribute to the premium costs. Insurance coverage for new employees will be effective the first of the following month after thirty days of employment.
DENTAL
Each Team member offers employees the opportunity to participate in a Group Dental Program.
VISION
Dependent upon the employee’s assignment, each Team member offers employees the opportunity to participate in a Group Vision Program.
LIFE AND ACCIDENTAL DEATH & DISMEMBERMENT
Each Team member provides life insurance and accidental death provision for all employees. Additional coverage is available and the rates are based on age and amount of insurance. Benefits through the supplemental plan are also available to spouse and children.
SHORT-TERM DISABILITY
Short-term disability insurance is provided after a period of total disability and will continue for up to 13 weeks. Ninety to 100% of the costs are borne by individual Team members.
LONG-TERM DISABILITY
Each Team members offers long-term disability income insurance coverage after short-term disability of 13 weeks of total disability.
AFLAC
As an added benefit, each Team member offers supplemental income protection through AFLAC as a pre-tax benefit.
HOLIDAYS
The Team will be closed for official business in accordance with the client’s holiday schedule.
PERSONAL LEAVE / VACATION
Personal leave may be used for any purpose, including illness, medical appointments, family illness, court appearances, personal business, snow days, and facility closings. All full-time employees will receive five (5) days of Personal Leave at the beginning of each fiscal year. This equals to an additional week of leave for all full time employees. Employees who work under 40 hours per week, on a continuous basis, or 20 hours or more hours per week, shall earn Personal Leave in proportion to the amount of time scheduled to work. Advanced vacation leave may be granted, up to 3 days (24 hours), with supervisor approval. Accrued vacation can be used at any time, considering the schedule of the workgroup.
JURY DUTY
Employees shall be granted paid leave for jury duty or a court appearance when the request is accompanied by an appropriate subpoena or summons and proof of service (obtained from the clerk of the court) provided that the employee is not a plaintiff or a defendant in the case. The amount of leave granted is limited to the time required to serve as a juror or witness plus reasonable travel time (not to exceed one week). Remuneration will be the difference between company pay and jury pay.
MILITARY LEAVE OF ABSENCE
The Company grants a short-term Military Leave of Absence with differential pay to full-time employees who have official orders requiring the participation in a training encampment, naval cruise, etc., upon the employee’s presentation of the orders. Differential pay will be paid to employees for a period of ten (10) working days per year after one (1) year of service. Requests shall be supported by a copy of the official military orders.
BEREAVEMENT LEAVE
Employees bereaved by the death of a member in their immediate family will be granted a day (8 hours) off from work to attend funeral services, without loss of pay.
PRE-TAX SAVINGS PLAN (401k)
Each Team member offers a substantial 401(k) plan.
EDUCATIONAL ASSISTANCE
Each Team has their own policy regarding educational assistance. Assistance is typically provided for full-time employees enrolled in an accredited institution for pre-approved job-related courses. This policy does not provide reimbursement when to do so would duplicate payment of specific items reimbursed or reimbursable to the employee under a state, federal, or other educational assistance program. Employees who leave their position prior to one year from completion date of the course are required to reimburse all costs incurred.
PROFESSIONAL SEMINARS
The Team encourages the professional development of employees through the attendance and participation in approved meetings, seminars, etc., which are directly related to the Team’s operation and objectives, which will place employees in a position to improve their job performance. Prior approval is required.
EMPLOYEE REFERRAL
All employees are encouraged to recommend candidates for employment. For this assistance, the Team offers special financial rewards if the candidate is hired.
PAY/DAY DIRECT DEPOSIT
Payday is every two weeks or bi-monthly. Direct deposit is provided for all employees and is encouraged.
